Your medical records are important. I can provide
better service if I have them before seeing you in the office. In order
to make the transfer process as simple as possible, please follow these
simple steps:
1. Click here to download
the form necessary to authorize Randolph Internal Medicine to
transfer a copy of your records to our practice. It may take up to
10 days for Randolph to get the records to us.
2. Complete the form and send it to Parker Internal
Medicine at the above address so we can deliver this to Randolph.
We can then contact you after we receive your records and set up an
appointment.
3. If you are in the Randolph Internal Medicine
office, you can ask them to give you the form to complete or you could
walk down the hall and we can initiate the process for you.
If you are having problems, however, please do not delay seeking evaluation
until I have your records. At the time of an office visit, we can initiate
this process as well. However, your medical history and laboratory results
from the prior years will not be available based on my memory alone!
